Enter your legal name and Social Security Number on your return and any correspondence. NOTE: On a joint return, list the names and the Social Security Numbers on Form IT-540 in the same order that you listed them on your federal return. A return for which a payment is due must be mailed to P.O. Box 3550, Baton Rouge, LA 70821-3550. Print the last four digits of your Social Security Number on your check or money order. DO NOT SEND CASH.
