OLT has created the “OLT One Account” for our customers starting in 2018. Click on “start now” at www.OLT.com or select to create an account. Once you have created your account, you will be able to log into one account and have access to 2018, plus the prior years of 2017, 2016, and 2015. If you need access to 2014, please contact customer service. Years prior to 2014 are no longer available on the OLT site.
Within the OLT One Account, click on the year that you are needing to view/print forms. You can go directly to that years' account or there is a view/print link on the year page as well.

Please make sure you do not have a filter or block on your email. You can also add email@example.com and firstname.lastname@example.org to your e-mail address book.
Some users may have trouble if they do not have the latest version of Adobe Acrobat installed. On a rare occasion, very few users still have trouble even after reinstalling Adobe Acrobat. If that happens, try right-clicking on the link that opens the Adobe Acrobat file and `Save Target As` onto your desktop. Then, try double-clicking the icon that is on your desktop - this will open the file outside of your web browser, and should allow you to view and print your forms.
